All copies made for genealogical purposes will be certified copies – no exceptions.

Birth  &  Marriage  and Death  Certificates for Genealogy

You may receive birth certificates for genealogical purposes if they are 75 or more years old.

Marriage records may be received for genealogical purposes if 50 or more years old.

You may receive death certificates for genealogical purposes if they are 20 or more years old.

Fees  –

Birth and Marriage – One certified copy is $17. Additional copies of same certificate are $9 each.                    

Death – One certified copy is $17. Additional copies of same certificate are $13 each.                                

To order a genealogical copy of a birth certificate, marriage certificate or death certificate by mail: 

1. Print and complete the entire Genealogy Request Form, available here.
2. Send a legible/readable copy of your driver’s license/photo I.D.
3. Send a personal check or money order for required fee payable to Fulton County Clerk.
4. Mail all of the above to: Fulton County Clerk, PO Box 226, Lewistown, IL 61542
